Medicare Part D
Extra Help & Part D Costs in West Chester, Pa. (2027)
WEST CHESTER, Pa. — Prescription drug costs remain one of the biggest worries for older adults on Medicare. The Part D out-of-pocket cap is $2,100 in 2026 and $2,400 in 2027. For people with limited income, there's a federal program that can lower those costs even further — and many who qualify have never signed up.

- The Part D out-of-pocket cap is $2,100 in 2026 and $2,400 in 2027, and the standard Part D maximum deductible is $700 in 2027.
- Extra Help, also called the Low-Income Subsidy, is a federal program that works alongside a Part D drug plan to lower premiums, deductibles, and copays for people with limited income and resources.
- Since 2024, everyone who qualifies for Extra Help gets the full subsidy, since the old partial-benefit tier was eliminated.
- Qualifying for Extra Help also gives you a Special Enrollment Period, letting you change drug plans outside the normal enrollment windows, and applying is free through Social Security or your local SHIP program.
It's called the Low-Income Subsidy, better known as Extra Help. Run by the Social Security Administration and Medicare, it helps pay Part D premiums, deductibles, and copays for prescription drugs. Here in West Chester, local advocates say awareness is still the biggest hurdle.
What Extra Help Actually Does
Extra Help is not a separate insurance plan. It works alongside a Medicare Part D drug plan (either a standalone plan or the drug coverage built into a Medicare Advantage plan). Once you qualify, the program reduces or eliminates your monthly Part D premium (up to a benchmark amount), wipes out the Part D deductible, and caps what you pay at the pharmacy counter for covered generic and brand-name drugs.
The Part D out-of-pocket cap is $2,100 in 2026 and $2,400 in 2027. The standard Part D maximum deductible is $700 in 2027. Extra Help enrollees generally pay far less than those figures.
Who Qualifies in 2027
Eligibility is based on income and resources. According to the Social Security Administration, you may qualify for the full Extra Help benefit if your income is at or below 150% of the federal poverty level and your resources (things like savings, stocks, and bonds) fall under the program's limits. Your home, your car, and personal belongings do not count as resources.
Starting in 2024, Congress expanded Extra Help so that everyone who qualifies now receives the full subsidy — the older "partial" tier was eliminated. That change remains in place for 2027.
You are automatically enrolled in Extra Help if you:
- Have both Medicare and full Medicaid,
- Get help from a Medicare Savings Program, or
- Receive Supplemental Security Income (SSI).
Everyone else has to apply.
West Chester Is in Chester County
West Chester sits in Chester County, where Medicare beneficiaries can choose from a wide range of stand-alone Part D drug plans and Medicare Advantage plans that include drug coverage. The number of options changes each year, so it's worth reviewing your plan during the Annual Enrollment Period (October 15 – December 7) to make sure your current coverage still fits your medications.
If you qualify for Extra Help, you also get a Special Enrollment Period that lets you switch drug plans outside the usual windows — a helpful safety net if your prescriptions change.
Common Mistakes to Avoid
A few missteps trip up otherwise eligible people:
- Assuming you make "too much." The income limits are higher than many people think, and not every dollar counts (for example, in-kind help from family for food or housing is treated differently now).
- Not applying because you already have a drug plan. Extra Help layers on top of an existing Part D plan. You don't have to drop coverage to apply.
- Confusing Extra Help with Medicaid. They're related but separate. You can qualify for Extra Help without qualifying for Medicaid.
- Forgetting to reapply after a life change. If your income drops — say, after a spouse passes away or you stop working — you may now qualify even if you didn't before.
How to Apply
There is no fee to apply, and you can do it three ways:

Pennsylvania's State Health Insurance Assistance Program (SHIP), known here as APPRISE, offers free, unbiased one-on-one counseling and can walk you through the application. Chester County residents can reach APPRISE through the county's Department of Aging Services.
