Garfield Heights Medicare: 2027 Part D cap & gap

Garfield Heights, Ohio — If you opened a letter from Social Security this fall and saw an unfamiliar charge tacked onto your Medicare premium, you're not alone. As people across Cuyahoga County weigh their 2027 coverage, higher earners are getting a fresh look at how income can raise what they pay for Part B and Part D — a rule known as IRMAA.

Key takeaways

  • IRMAA is an extra charge added to Part B and Part D premiums for people whose income is above a certain level, not a penalty for wrongdoing.
  • For 2026, the surcharge starts above $109,000 in income for a single filer and roughly $218,000 for a married couple filing jointly, with higher tiers costing more.
  • Your IRMAA amount is based on your income from two years earlier, so a 2025 tax return will determine what you pay in 2027.
  • If your income has dropped due to a life-changing event, you can file Form SSA-44 with documentation to ask Social Security to recalculate your IRMAA.

Here's what that means, who it hits, and what to do if you think the number is wrong.

What IRMAA actually is

IRMAA stands for the Income-Related Monthly Adjustment Amount. It's an extra charge added on top of the standard Part B premium and, separately, on top of your Part D drug plan premium. It isn't a penalty for doing anything wrong — it's simply how Medicare asks people with higher incomes to pay a larger share of program costs.

Most people on Medicare do not pay IRMAA. It only kicks in once your income crosses a specific line.

The income thresholds that trigger IRMAA

For 2026, IRMAA surcharges begin above $109,000 in income for a single filer, and above roughly $218,000 for a married couple filing jointly. Below those numbers, you pay the standard Part B premium and only your plan's regular Part D premium — no add-on.

Above those numbers, the surcharge climbs through several tiers. The higher your income, the higher the extra amount, on both Part B and Part D. Social Security posts the full tier chart each year, and Medicare.gov keeps an updated table.

One important detail: Medicare looks at your modified adjusted gross income (MAGI) from two years ago. So your 2027 IRMAA will be based on your 2025 tax return. That two-year lag catches a lot of people by surprise, especially anyone who recently retired, sold a home, or took a large IRA withdrawal.

How you'll be billed

That split trips people up. If you ignore the Part D IRMAA bill thinking it's a scam or a duplicate, you can lose your drug coverage.

What's new for 2027 that interacts with this

Even for people paying IRMAA, the drug side of Medicare looks very different than it did a couple of years ago:

IRMAA doesn't change any of these limits — a high earner still gets the same cap ($2,100 in 2026, $2,400 in 2027). But higher-income beneficiaries pay a larger monthly premium to be in the program.

If your income has dropped, you can appeal

This is the piece most people don't know about. If a "life-changing event" caused your income to fall since that two-year-old tax return, you can ask Social Security to recalculate.

Qualifying events include:

You file Form SSA-44 with documentation. If Social Security agrees, they'll use your more recent income instead. It's worth doing — the surcharges are real money every month.

Where Garfield Heights fits in

Garfield Heights is in Cuyahoga County, where Medicare beneficiaries choose from a wide range of stand-alone Part D drug plans and Medicare Advantage plans that include drug coverage. IRMAA works the same either way: whether you have Original Medicare with a stand-alone Part D plan, or a Medicare Advantage plan with drug coverage built in, the Part D surcharge still applies if your income is above the threshold.

The Annual Enrollment Period runs October 15 through December 7, and the Medicare Advantage Open Enrollment Period runs January 1 through March 31. Those windows are when you can change your drug or Advantage coverage — but they do not change your IRMAA. That's set by your tax return, not your plan choice.

Higher-income surcharges

IRMAA income brackets

If your income is above a threshold, you pay an income-related surcharge (IRMAA) on top of your Part B and Part D premiums. The 2027 brackets and surcharge amounts are set by CMS and are typically published later in the fall along with the Part B figures; the chart below reflects 2026.

Frequently asked questions

Does IRMAA ever go away on its own?
Yes. Because it's based on your income from two years ago, a drop in income will eventually work its way through. If your 2025 income was high but your 2026 income is lower, your 2028 IRMAA should reflect that automatically.
I just got a letter saying I owe a Part D surcharge, but my plan already bills me. Is this a mistake?
Probably not. Part D IRMAA is billed by Medicare, separately from your plan's regular premium. Both are legitimate. Call 1-800-MEDICARE if the letter looks off.
I'm still working past 65 and earning well. Should I delay Medicare to avoid IRMAA?
Not usually. IRMAA is recalculated every year, and delaying Medicare when you have other creditable coverage is a decision with a lot of moving parts. Talk to your local SHIP counselor before deciding.
